Terms and Conditions of Sales and Returns
1. Online Orders
- All orders placed prior to 2PM CST Monday-Friday will generally ship the same business day.
- Orders placed after 2PM CST are generally released for shipment on the next business day.
- A credit card payment is required to process all online orders.
- You will receive an order confirmation and tracking information as soon as your order is processed.
Orders are generally shipped via FedEx. Optimum Solutions Group LLC and shipping partners are not responsible for late arrivals due to poor weather conditions, incorrect shipping information provided by the customer or failed delivery attempts. Unless stated otherwise, delivery dates are only estimates and not guaranteed.
Terms of Return and Exchange
Products not listed on the Exclusion List below, can be returned or exchanged only within 30 days of purchase. Products must be in their original, unopened, unmarked, unsterilized and undamaged packaging with all factory seals in tact.
- Equal value exchanges will only be accepted within 30 days.
- All refunds within 30 days will incur a 15% restocking fee and return shipping fee (if applicable). Refunds will be made to the original payment method only after the products have been received and inspected.
- Exchanges or refunds after 30 days will not be accepted.
Products that do not qualify for returns/exchanges:
- Product(s) requiring special handling, such as refrigeration.
- Product(s) financed through a third party.
- Product(s) obtained through a third party.
- Returns of promotional purchases or discounted sets.
- Shipping charges.
Return / Exchange procedure
- Call our office at 630-705-1002 to acquire a Return Number. Please be ready to provide us with product(s) number and product(s) lot number.
- Write the Return Number on a copy of the corresponding invoice and place it in the package with the product(s) to be returned/ exchanged. Filled and signed by the doctor Return/ Exchange Form must be also included.
- Package item(s) in durable packaging to avoid damage or loss. Items missing from the package or damaged due to improper packaging will not qualify for any returns/exchanges.
- Return package must be shipped by the customer via a traceable method. Alternatively, you can obtain a return label via email (return shipping charges apply).
